Tailored Care Management
Tailored Care Management (TCM)
In 2022, Community Alternatives, Inc. dba Community Choices, Inc. was certified through state of North Carolina by National Committee for Quality Assurance (NCQA) to become a Tailored Care Management Agency. We chose to be certified in substance use and mental health. This was a new service area for North Carolina and it went live statewide in December of 2022.
Tailored Care Management works to coordinate care across physical health, behavioral health, and the social system of the member. By integrating all of these systems, the goal is to improve the members overall health and satisfaction and reduce the cost of health/behavioral healthcare.
Tailored Care Management (TCM) is a free service that pairs an individual with a specially trained health care worker who knows the health care system. This person is called a Tailored Care Manager. The TCM’s go through extensive training when they are hired and must complete a month long worth of training before they are approved to work with members.
Tailored Care Managers are trained to help people with serious mental illness, severe substance abuse disorder, intellectual/developmental disabilities or traumatic brain injury. CAI/CCI will only work with individuals who are diagnosed with serious mental illness or severe substance abuse disorder.
A Tailored Care Manager can become the members partner and help them to navigate the health care system. They are there to help with things such as:
- Find doctors who take Medicaid and have openings
- Arrange free rides to appointments
- Help get supports for school or work
- Identify resources for housing and food
- Help members manage chronic conditions, like diabetes
- Be the main point of contact for all things Medicaid
Tailored Care Management works with individuals who have NC Medicaid.
CAI/CCI operates two teams: Charlotte and Winston-Salem. Our Tailored Care Management Program is supervised by a Clinical Manager. Each team consists of a Care Manager Supervisor and up to 8 care managers and 4 extenders or care manager assistants.
